Understanding the Plan Terms and Conditions

• This dental discount plan is distinct from dental insurance and cannot be used in conjunction with any other dental insurance.
• The plan becomes effective on the date you sign the agreement, and it automatically renews one year after the agreement date.
• Exams, x-rays, cleanings, and fluoride treatments need to be conducted within the enrollment year and cannot be carried over to the subsequent year.
• Payment is required at the time of service for any procedures not fully covered by this plan.
• Failure to pay the full duration membership fees of the plan may result in being billed usual and customary fees for all services rendered, including those already received, and being required to pay the remaining months’ fees in a single lump sum.
• This plan is exclusively accepted at Apple Dentistry Arlington. Consequently, if you are referred to a specialist, they will not extend these discounts.
• If dental treatment is required as a result of any injury involving a lawsuit or other forms of outside medical care, disability, or workers’ compensation insurance, this discount plan cannot be utilized.
• This plan is not transferable, and family members cannot serve as substitutes for each other.
• This plan is non-refundable. Patients are not eligible for refunds if they opt not to utilize their dental plan after the initial 30 days following enrollment.
• Members are required to maintain membership for the entire duration of Clear Correct® treatment.
• This plan exclusively covers dental services, products are not included.
• A fee, based on the length of the appointment, may apply for any broken appointments without a 24-hour advance notice.
• Rates may be subject to annual changes.
